Terms and Conditions Dental Practice
Applicability
1.1 These general terms and conditions apply to all treatments, services, and agreements between the dental practice and the patient.
Appointments and Cancellations
2.1 The patient is required to cancel appointments in a timely manner, at least 48 hours before the scheduled appointment, excluding weekends (Saturday and Sunday).
2.2 Failure to cancel on time or non-attendance may result in the dental practice reserving the right to charge fees.
2.3 Fees for failure to cancel in a timely manner amount to 10 euros per 5 minutes for missing a regular appointment and 75 euros for a sedation intake.
2.4 For failure to cancel a sedation treatment in a timely manner, we refer to the signed treatment agreement.
Payments
3.1 The initial visit is paid for upfront (which can be later reimbursed by the insurer), and then all subsequent transactions are directly processed through the insurance.
3.2 The dental practice assigns debts to Infomedics for collection; payment must be made directly to Infomedics.
3.3 Failure to meet payment obligations results in immediate default, with the practice reserving the right to suspend further treatment.
Rates and Reimbursements
4.1 Treatment rates are determined according to the applicable fee schedule, in compliance with the regulations of the Dutch Healthcare Authority (NZa).
4.2 The patient is responsible for verifying reimbursements with their health insurer.
Liability
5.1 The dental practice is not liable for damages of any kind arising from the dentist relying on incorrect and/or incomplete information provided by the patient.
5.2 The dental practice is not liable for damages resulting from the improper follow-up of advice after a treatment.
5.3 Liability is limited to the amount covered by professional liability insurance, plus the deductible.
Complaints Procedure
6.1 Complaints about treatment should be reported in writing to the dental practice as soon as possible, but no later than 48 hours after discovery, excluding weekends (Saturday and Sunday).
6.2 The dental practice will handle complaints as quickly as possible and to the best of its ability. Filing a complaint does not delay the payment term.
Privacy and Data Protection
7.1 The dental practice handles the patient’s personal data carefully and complies with applicable privacy laws.
7.2 Personal data is only processed for the purpose for which it was provided.
Changes to General Terms and Conditions
8.1 The dental practice reserves the right to modify these general terms and conditions. Modified conditions take effect from the date of publication.
8.2 Patients will be informed of changes to the general terms and conditions.
Intake, Examinations and the Non-Continuation of Treatment
9.1 Prior to any treatment, it may be necessary to carry out an intake consultation, (additional) examinations and/or diagnostics, including radiographic examinations. These form an integral and independent part of dental care. In accordance with the regulations and performance descriptions of the Dutch Healthcare Authority (NZa), these services are always billed separately.
9.2 If, following assessment of the medical questionnaire, the intake consultation, additional examinations, or on the advice of the treating dentist or anesthesiologist, it is decided that the intended treatment cannot (safely) take place, the costs of the care, examinations, diagnostics and/or intake already performed remain fully payable.
9.3 Whether a treatment can proceed depends in part on medical circumstances that may only become apparent during or after the intake or examination. The practice cannot accept liability for this, and patients cannot derive any rights to reimbursement of costs already invoiced.
9.4 Information provided by the practice via the website, brochures or other general communications is intended solely for general informational purposes. No rights can be derived from this information. The final assessment of eligibility for treatment and the treatment options to be followed will always be determined by the treating dentist and/or anesthesiologist, based on a full medical evaluation.
By scheduling an appointment with the dental practice, the patient agrees to these general terms and conditions.
